Documents Required For Aadhaar Card Application

To enrol for an Aadhaar Card, you need to submit a proof of identity and proof of address. The identity proof must contain your photograph and name. The address proof should have your photograph and name. Also, you need to provide proof of age/date of birth. If you lack identity proof or address proof, then you will require relevant documents of an Introducer or Head of the Family. The Head of the Family has to provide proof of relationship with you.

NOTE: Do remember that you have to submit the original copy of the documents you wish to submit. The staff at the Aadhaar enrolment centre will scan and return the papers to you. There is no need to carry a printout of Xerox copy of the original papers.

Some of these documents required for Aadhaar Card application are also needed to request for a correction/update. The papers are required particularly for the requested change. For instance, you want to change your existing address with another. In such a case, you need to provide the proof of a new address when submitting the update request, either online or offline.

1. Proof of Identity Documents

The paper must contain your photograph and name. It must be government-recognized proof. You need to submit any one of the acceptable identity proofs. Below-mentioned are the documents the Aadhaar Seva Kendra accepts as an ID proof:

  • Passport
  • Driving licence
  • PAN card
  • Voter ID
  • Ration card/PDS photo card
  • Photo credit card
  • NREGA Job Card
  • Photo Bank ATM Card
  • Arms Licence
  • Kissan Photo Passbook
  • ECHS/CGHS Photo Card
  • Pensioner Photo Card
  • Freedom Fighter Photo Card
  • RSBY Card
  • Bhamashah Card
  • Marriage Certificate with photograph
  • Photo ID Card provided by a recognised educational institution
  • SC/ST/OBC Certificate comprising photograph
  • Certificate of Identity containing photograph. It should be on a letterhead and issued by a Gazetted Officer or Tehsildar
  • Certificate of Identity that has photographs. It should be on a letterhead and issued by an MP/ MLA/MLC/Municipal Councillor.
  • Photo ID Card that is government-issued/ PSU-issued Service Photo ID Card
  • In case of a name change, submit a Gazette notification
  • Department of Posts issued Address Card with photograph and name
  • SSLC book containing photograph
  • Certificate from the head of a recognised educational
    institution/matron/superintendent/warden
  • For rural areas - Certificate of Identity with photograph. It should be issued by the head of the Village Panchayat
  • Disability ID card or handicapped medical certificate issued by the State/Administrations/Union Territory Government

2. Proof of Date of Birth (DOB) Documents

The Date of Birth proof should be government-recognized. You have to submit any one of the acceptable DOB proofs. Below-mentioned are the documents the Aadhaar Seva Kendra accepts as a DOB proof:

  • Birth Certificate
  • PAN Card
  • Passport
  • State/Central Pension Payment Order
  • PSU or government issued Photo ID Card with date of birth
  • SSLC Book/Certificate
  • Ex-Servicemen Contributory Health Scheme Photo Card
  • Photo ID card with DOB. It should be issued by a recognised educational institution
  • A Date of Birth Certificate on a letterhead. It must be issued by a Group a Gazetted Officer
  • I-Card/Certificate with DOB and photograph. It must be signed and issued by a Government authority
  • Marksheet provided by an University or a Government Board
  • Central Government Health Scheme Photo Card

3. Proof of Address

The document for Proof of Address should contain your name and address. It can also be a property you stay at under someone’s care. Here are the acceptable forms of address proofs that you can submit:

  • Ration Card
  • Water bill/Telephone Landline bill/Credit Card Statement/Gas connection/Electricity bill (issued in the last 3 months)
  • Passport
  • School ID Card
  • Property Tax Receipt (issued in the last 1 year)
  • Driving Licence
  • Signed Letter on letterhead with photograph. This document must be issued a bank
  • Registered Lease/Sale/Rental Agreement
  • Passbook/Bank Statement
  • Arms Licence
  • ECHS/CGHS Card
  • NREGA Job Card
  • Bhamashah Card
  • Signed Letter on letterhead with photograph. This should be issued by a registered company
  • Insurance Policy
  • Income Tax Assessment Order
  • SSLC Book with photograph
  • Pensioner Card
  • Freedom Fighter Card
  • Kissan Passbook
  • Certificate of address on a letterhead. This must be issued by Head of Institution/ Superintendent/Warden/Matron of a recognised orphanage or shelter home Government or PSU-issued Photo ID Card
  • Government or PSU-issued Photo ID Card
  • Voter ID
  • Certificate of Address with photo on a letterhead. This must be issued by MLA/MP/Tehsildar/Gazetted Officer
  • Passbook/Post Office Account Statement
  • Passport of the spouse
  • Photo ID containing address/Signed Letter with photograph. This must be issued by a recognised educational institution
  • Disability ID card or handicapped medical certificate issued by the State/Administrations/Union Territory Government
  • Vehicle Registration Certificate
  • For rural areas – Certificate of Address. This has to be issued by the head of Panchayat or a similar authority
  • Allotment letter of accommodation in the last 3 years. This should be issued by State/Central Government
  • Address Card with a photograph. This should be issued by Department of Posts
  • Passport of the parent in case the applicant is a minor
  • ID Card that should be provided by a recognised educational institution
  • Caste and Domicile Certificate with a photograph issued by the State government
  • Marriage Certificate with address issued by the government
  • Certificate of address with a photograph on letterhead issued by the Municipal Councillor

4. Proof of Relationship Documents

The Proof of Relationship paper must contain details of Head of the Family (if required). You can submit any of the documents that are government-recognized. The acceptable papers for Relationship proof are:

  • Family entitlement document. This should be issued by Central or State Government
  • Army Canteen Card
  • Marriage Certificate issued by the government
  • Passport
  • Address Card with photograph and name. This must be issued by the Department of Posts
  • For rural areas – Certificate of Identity with photograph. This document must state the relationship with the head of family. The paper needs to be issued by the head of the village Panchayat
  • Bhamashah Card
  • State Government/ECHS/ESIC/CGHS Medical Card
  • Pension Card
  • Certificate of Identity with photograph on a letterhead. This paper must be issued by a Municipal Councillor/MP/MLA/MLC/Gazetted Officer
  • PDS Card
  • NREGA Job Card
  • Birth Certificate must be provided by the Registrar of Birth, Municipal Corporation, or any other notified Government local body such as Taluk, Tehsil, etc.
  • In case of birth of a child- discharge slip/card provided by a government hospital
